How to Make a Claim
Any employee with a life-threatening or serious injury must immediately be taken to the nearest hospital emergency room. If an ambulance is needed, call 911. For injuries that are not life threatening or serious, follow these procedures:
- Complete an accident investigation report.
- Fax to Propel HR at (864) 232-0178
ATTENTION: Karyn Mulligan
- Send employee to the nearest clinic or doctor’s office.
- Notify Propel HR the day the employee returns to work.
- If accident occurs after hours (8:30-5:30), fax the report the following day and send the employee to the nearest EMERGENCY ROOM OR CLINIC.
- If more information is needed for the insurance carrier, Propel HR will be in contact with the supervisor. If there are any medical claims, please send them to:
